Privacy statement

When you contact via this website, you leave personal data. Here you can read which data the form records, what they are used for, and how you can ask a privacy question. For questions about your treatment or medical file, please contact the practice that is treating you. The practice details on this page help you find the right point of contact.

Your practice and the responsible organization

These websites present collaborating dental care practices. Collaboration does not mean that every practice has access to every patient file or that all practices are one legal organization. Orthodontie Helmond and Implantoloog Helmond are parts of the practices in Helmond, not independent visiting locations on this website. When inquiring about privacy, mention which practice you were treated at or through which website you made a request.

The practice you contact is responsible for your data; its name and Chamber of Commerce (KvK) number are shown in the box on this page. Editorial status before launch: the agreements with service providers for hosting and email still need to be finalised. This version describes the current website and is not yet a complete statement about all of the practices' systems.

What data the contact form records

  • Your name, email address, chosen reason for contact, and message.
  • Your phone number and chosen treatment, only if you fill in these optional fields.
  • The practice to which your request is linked, the time of submission, the status of the request, and the checked privacy box.

This data is intended to read your question, contact you, and discuss a possible appointment with you. A completed form is not a confirmed appointment and not an entry in a medical file. The box next to the form does not give consent for advertising and is not a general consent to share medical data with other organizations.

Only fill in what is necessary to make contact. Do not send BSN, identity documents, medical files, or extensive information about your health via the form. Ask the practice for an appropriate, secure way if medical information is needed. In case of emergency, do not use the form, but the telephone contact route of your practice.

Goals and grounds

A request for an appointment can be processed to take steps towards a treatment agreement at your request. In the case of a general question, the interest in answering your message can be a ground, provided that your privacy interests are carefully weighed. Medical data has extra protection: in addition to a regular AVG ground, an allowed exception for health data must apply. Checking the form does not waive these rules.

For data that the practice needs for treatment, file management, or administration, the treatment agreement and legal obligations may apply. Where consent is the basis, you can withdraw that consent again. This does not make earlier lawful use unlawful retroactively. A legal retention obligation also does not automatically expire upon withdrawal.

Access, technical services, and security

Requests are stored in the website's database. Access to requests in website management is limited to network administrators; text editors do not receive this access due to their editorial role. The technical supplier of the database processes the storage. The final hosting and email setup and the associated agreements must be established before going live, including any transfer outside the European Economic Area and the safeguards for that.

The website contains a hidden control field and a temporary counter per IP address to limit repeated submissions. That counter is not a marketing profile. For loading the website, the technical infrastructure also processes connection data. Which server logs are retained and for how long must be checked with the final hosting. A privacy statement alone does not prove that all security and organizational agreements are in place.

Maps and local storage

Google Maps only loads after you give your consent. When it loads, Google receives your IP address and browser data, among other things. The map choice is stored in your browser under practice-external-maps-v1. You can withdraw this consent via Cookie settings at the bottom of the page; the map then disappears. This does not delete any data Google received earlier. The website contains no advertising trackers. More explanation can be found on the cookie page.

Visitor statistics

To see how our website is used, we measure visits with Vercel Web Analytics. This service is provided by Vercel Inc. from the United States, which also hosts our website. No cookies are placed. A visit is only recognised by a code calculated from the request, which expires after 24 hours; we cannot identify anyone with it.

We see aggregated figures: the page visited, the referring website, the location at country, region or city level, the device type, the operating system and the browser. We also count clicks on a phone number and submitted contact forms, recording only the type of request and never your name or contact details. We do this on the basis of our legitimate interest in keeping the website clear and accessible. The figures remain visible for twelve months.

Retention and deletion

A single website request is not automatically a medical record. We keep messages sent through the contact form for as long as needed to handle your question, and delete them no later than twelve months afterwards. If you become a patient, the practice adds the information needed for your treatment to your patient record. The periods below apply to that record.

A medical record has a statutory retention period of twenty years from the last change, or longer if your care requires it (Dutch Medical Treatment Contracts Act, WGBO). You can ask for your record to be destroyed; whether this is possible depends on the applicable rules. The practice keeps invoices and other financial records for seven years, as required by tax law. These periods do not apply to individual messages from the contact form.

Exercising your privacy rights

You can request access, correction, or deletion of your personal data. Depending on the processing, you can also request restriction or transfer, object, or withdraw consent. These rights are not unlimited; for example, the rights of others and legal obligations may be relevant. A refusal must be explained.

Use the practice contacts on this page and mention that it concerns a privacy request. Describe which data or processing you mean, without directly sending a copy of your identity document. The practice must be able to verify your identity carefully. In principle, a response to your request must be made within one month. For complex or multiple requests, a maximum of two additional months may be needed; you must be informed about that extension and the reason within the first month.

If you disagree with the processing or handling of your request, you can file a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens). A conversation with the practice may help, but it does not take away this right. Through the source links on this page, you will find further explanations about your rights and medical files.
